(c) Permission to use quotations from the Interview (or excerpts from such quotations), the film, the photograph(s), the cassette(s) or the reproduction(s) of mine and/or the recording of my voice in whole or in part in their publications, newspapers, magazines and other print media, television, radio and electronic media (including the Internet), theatre media and/or education and awareness-raising mailings. As with everything, do your due diligence. Know what you want to do with your video and interviews now, but don`t limit yourself to your current situation either. Be frank and transparent about your expectations in your video and interview release consent form for you and your talent and start from there. (a) permission to interview, film, photograph, record or otherwise make a video of me and/or record my voice; Consent and release forms come in many shapes and sizes, but no matter what details or type of project you`re working on, you should always have stacks of consent and release forms on deck when mixing media and talent. Consent and release forms are given to your talents (interviewees, models, actors, etc.) and allow you to use their image (in video or photo form), audio and words in your production. When creating your interview and sharing videos, consent, including the time limit and expiration of your permissions, is crucial. Unless you have been asked to set a specific period for the validity of the consent form, you must always indicate in writing that the release form is valid indefinitely for current and future use. This means that no matter what situations may change in the future, you have permission to use the appropriate multimedia form for as long as you want.

To cover all your bases, your agreement on permission to record videos should include a clause on commercial and non-commercial rights. Non-commercial filming is usually used for educational, artistic or personal purposes. Non-commercial media are not used for advertising or profit-making purposes. Before you can jump headfirst into shooting your (non-actor) subjects, there`s one important point they need to fill out before they ride: sign a video sharing form. While all video broadcast forms vary slightly in content and word choice, here are some important points to include in your video and interview release consent form. There is no single template, so do some research and be sure to include all clauses related to your project`s current and future requirements. Disclaimer: Below is an example of a template for a video interview consent and release form (download below). Please use it as a guideline when creating a release form for your own production. We cannot guarantee that this template will cover all legal requirements that are unique to your project! I, __________ This consent is given without compensation, regardless of the amount of revenue generated by the content. Once the document is signed, the entity or organization is not required to obtain the consent of the individual when using their name or image in publications, advertising or other media companies (including the Internet).

A parent or guardian must sign the consent form if the person is a minor. Signing a consent form to share a video and an interview before filming begins will prevent the person from coming to you and saying that they want the use of their image removed from the project. Essentially, a video broadcast form helps avoid further legal complications that may arise in the future.
